The level of medical care to patients and the success of the implementation of the national project "Health" depend on the health status and doctor's professionalism. Modern healthcare reform is being implemented by optimizing costs, merging medical organizations, closing ineffective hospitals, expanding the use of high-tech care and informatization of the doctor's activities. All this makes it necessary to assess the health of doctors. Scientists have studied the quality of life and identified the professional burnout of doctors in children's clinics (...) in different age groups. Primary care pediatricians have a higher quality of life score when compared to the standard [1]. However, all groups have a high level of professional burnout. The pre-retirement group of doctors has more unfavorable profile. They are highly professional specialists but may experience emotional discomfort because the pension system has been reformed. This situation requires the introduction of preventive work with doctors of this age as a group with great potential and capable of professional longevity. (shrink)

The contemporary view of the fundamental role of time in physics generally ignores its most obvious characteric, namely its flow. Studies in the foundations of relativistic mechanics during the past decade have shown that the dynamical evolution of a system can be treated in a manifestly covariant way, in terms of the solution of a system of canonical Hamilton type equations, by considering the space-time coordinates and momenta ofevents as its fundamental description. The evolution of the events, as functions of (...) a universal invariant world, or historical, time, traces out the world lines that represent the phenomena (e.g., particles) which are observed in the laboratory. The positions in time of each of the events, i.e., the time of their potential detection, are, in this framework, controlled by this universal parameter τ, the time at which they are generated (and may proceed in the positive or negative sense). We find that the notion of thestate of a system requires generalization; at any given τ, it involves information about the system at timest(τ) ≠ τ. The correlation of what may be measured att(τ) with what is generated at τ is necessarily quite rigid, and is related covariantly to the spacelike correlations found in interference experiments. We find, furthermore, that interaction with Maxwell electromagnetism leads back to a static picture of the world, with no real evolution. As a consequence of this result, and the requirement of gauge invariance for the quantum mechanical evolution equation, we conclude that electromagnetism is described by a pre-Maxwell field, whose τ-integral (or asymptotic behavior as τ → ∞) may be identified with the Maxwell field. We therefore consider the world of events in space time, interacting through τ-dependent pre-Maxwell fields, as far as electrodynamics is concerned, as the objective dynamical reality. Our perception of the world, through laboratory detectors and our eyes, are based onintegration over τ over intervals sufficiently large to obtain an aposteriori description of the phenomena which coincides with the Maxwell theory. Fundamental notions, such as the conservation of charge, rest on this construction. The decomposition of the common notion of time into two essentially different aspects, one associated with an unvarying flow, and the second with direct observation subject to dynamical modification, has profound philosophical consequences, of which we are able to explore here only a few. Gauge invariance of a manifestly covariant relativistic quantum theory with evolution according to an invariant time τ implies the existence of five gauge compensation fields, which we shall call pre-Maxwell fields. A Lagrangian which generates the equations of motion for the matter field (coinciding with the Schrödinger type quantum evolution equation) as well as equations, on a five-dimensional manifold, for the gauge fields, is written. It is shown that τ integration of the equations for the pre-Maxwell fields results in the (...) usual Maxwell equations with conserved current source. The analog of the O (3, 1) symmetry of the usual Maxwell theory is found to be O (3, 2) or O (4, 1), depending on the space-time Fourier spectrum of the field. We argue that the structure that is relevant to the description of radiation in interaction with matter evolving in a timelike sense is that of O (3, 2). The noncovariant form of the field equations is given; there are two fields of electric type and one (divergenceless) magnetic type field. The Noether currents are studied, and some remarks are made on second quantization. (shrink)

Escalation of commitment causes people to continue a failing course of action. We study the role of construal level in such escalation of commitment. Consistent with the widely held view of construal level as a primed effect, we employed a commonly used prime for manipulating this construct in a laboratory experiment. Our findings revealed that the prime failed to produce statistically significant differences in construal level, which was measured using the Behavior Identification Form. Furthermore, there was no effect of the (...) prime on escalation of commitment, or on constructs that have previously been linked to construal level such as the perceived importance of feasibility considerations relative to desirability considerations, and the number of pros and cons that subjects can think of.Interestingly, however, subjects’ actual construal level scores on the BIF were found to significantly affect escalation. Specifically, our findings show that people with a low construal level are less willing to continue a failing project. This relation is mediated by the perceived importance of the feasibility of the project relative to its desirability. For people with a low construal level, the perceived importance of feasibility relative to desirability is higher, which in turn makes escalation of commitment less likely.Our findings are consistent with prior research suggesting that priming construal level may not always be effective. Thus, while construal level has typically been regarded as a state-like variable that can be primed, in this study, we show that construal level may also be considered as a trait. (shrink)

It has been pointed out to me that in my article on ‘The Augustan Rules for Dactylic Verse’ I misrepresented an observation of Maas as reported by Wilamowitz in his Griechische Verskunst, p. 53. Wilamowitz' words are: ‘Wenn Tibull und Ovid den Pentameter so bauen, dass die vorletzte Silbe betont wird, tun sie das nach dem Vorgange gleichzeitiger griechischer Epigrammatiker.’ This means, of course, that the Greek writers mentioned ended with a paroxytone word, not necessarily with a disyllable, as I (...) assumed it to mean, and I apologize for the oversight. The elements which every schoolboy learns on beginning Latin Verse Composition include a number of rules which seem arbitrarily designed to make the game harder. In hexameters, he is told, he must have a masculine caesura either in the third foot or in the second and fourth, and end normally with a disyllabic or a trisyllable; in pentameters he must end with a disyllabic; and in neither line may a single monosyllable stand at the end. Rarely, in my experience, is (...) any reason given him by way of redress, and he will search for one in vain in most of the school text-books, in introductions like Postgate's to Tibullus and Propertius, and in histories of Latin literature like Wight Duff's and Mackail's. This reticence may be due to the dissensions of experts on this subject and on the subject of Latin accentuation in general, but the theory that predominates in England, among those who hold a theory at all, explains so many of the phenomena that it deserves to be more widely and precisely known. The most detailed exposition of it is by E. H. Sturtevant, who summed up his analyses in a pair of articles in the Transactions of the American Philological Association in 1923–4. While his careful work is invaluable as marshalling the statistics and evidence, it errs on the side of excessive minuteness, and leaves room not merely for some additions, but for a different kind of treatment concerned less with bare statistics and more with poetic principles and historical development. Since Latin Verse Composition plays such an important part in English higher classical education, it seems desirable that a less technical and more accessible account should be available for English readers. Such an account I attempt to give here, keeping separate as far as possible the exposition and the consideration of the criticisms and rival theories that have been advanced. (shrink)

Some of the problems associated with the construction of a manifestly covariant relativistic quantum theory are discussed. A resolution of this problem is given in terms of the off mass shell classical and quantum mechanics of Stueckelberg, Horwitz and Piron. This theory contains many questions of interpretation, reaching deeply into the notions of time, localizability and causality. A proper generalization of the Maxwell theory of electromagnetic interaction, required for the well-posed formulation of dynamical problems of systems with electromagnetic interaction is (...) discussed, and some of the significance of recently found (classical) relativistic chaotic behavior is pointed out. Many results of a technical nature have been achieved in this framework; in this paper, some of these are reviewed, but I shall concentrate on a discussion of the basic ideas and foundations of the theory. Purpose: In the paper, I discuss how Ernst Glasersfeld worked as a scientist on the project, Interdisciplinary Research on Number (IRON), and explain how his scientific activity fueled his development of radical constructivism. I also present IRON as a progressive research program in radical constructivism and suggest the essential components of such programs. Findings: The basic problem of Glasersfeld's radical constructivism is to explore the operations by means of which we assemble our experiential reality. Conceptual analysis is Glasersfeld's way of (...) doing science and he used it in IRON to analyze the units that young children create and count in the activity of counting. In his work in IRON, Glasersfeld first conducted a first-order conceptual analysis of his own operations that produce units and number, and then participated in a second-order analysis of the language and actions of children and inferred the mental operations that they use to produce units and number. Further, Glasersfeld used Piaget's concept of equilibration in the context of scheme theory in a second-order analysis of children's construction of number sequences and of more advanced ways and means of operating in the traffic of numbers. Research Implications: The scientific method of first- and second-order conceptual analysis transcends our work in IRON and it is applicable in any radical constructivist research program whose problem is to explore the operations by means of which we construct our conceptions. Because of the difficulties involved with introspection, conducting second-order conceptual analyses is essential in exploring these operations and it involves analyzing the language and actions of the observed. But conceptual analysis is only a part of the research process because the researchers are by necessity already involved in creating occasions of observation. The "experimenter" and the "analyst" can be the same person or they can be different people. Either case involves intensive and sustained interdisciplinary thinking and ways of working if the research program is to be maintained over a substantial period of time as a progressive research program. Quincey quotes the opening lines of Catalepton 5 as, Ite hinc,-inanes, ite, rhetorum ampullae, inflata rhoso* non Achaico verba, and adds, ‘the second line is corrupt and no satisfactory emendation has been proposed’. The MS. readings are: rhorso B, roso Mu, om. in lacuna Ar. In face of these voces nihili many have fallen back on the rore of the Aldine edition of 1517. But this does not really help, for one (...) does not inflate with dew: orators are not dew-bags, but wind-bags. It occurred to me some years ago that what is needed is some word meaning breath or wind to go with inflata, and that in view of the rh in rhorso it was probably a Greek word which a scribe had failed to recognize. I conjectured οζ, and found subsequently that this had been proposed by K. Münscher in Hermes, xlvii , pp. 153–4. οȋζος used of any rushing sound, is applied to speech by Philostratus , and by Pollux . It is easy to see how rhoezo could degenerate into rhoeso-rhoso-roso. Purpose: One of my goals in the paper is to investigate why realists reject radical constructivism (RC) as well as social constructivism (SC) out of hand. I shall do this by means of commenting on Peter Slezak’s critical paper, Radical Constructivism: Epistemology, Education and Dynamite. My other goal is to explore why realists condemn the use of RC and SC in science and mathematics education for no stated reason, again by means of commenting on Slezak’s paper. Method: I restrict my (...) comments to Slezak’s paper and leave it to the reader to judge which, if any, of the reasons that I advance for these two states of affairs are not specific to Slezak’s paper. Other readers might not agree with my interpretations of Slezak’s paper, including Slezak himself, but I offer them after having worked with von Glasersfeld in interdisciplinary research in mathematics education for over 25 years. Findings: My findings are that Slezak: (1) rejects RC and SC on the basis of unjustified criticisms, (2) does not explore basic tenets of RC nor of SC beyond the unjustified criticisms, (3) rejects how SC and RC have been used in science and mathematics education, based at least in part on the unjustified criticisms, (3) dislikes how SC has been used in science and mathematics education, a dislike that fuels his rejection of any constructivism, and (4) doesn’t explore how RC has been used in scientific investigations in mathematics education. On the basis of these findings, I conclude that how epistemological models of knowing might be used in science or mathematics education would be better left to the educators who use them in interdisciplinary work. In his valuable contribution to the Fondation Hardt Entretiens of 1960 on Hesiod Professor La Penna dealt with the famous ‘theodicy of labour’ in Virgil, Georgics I. 118–59. He recalled that, whereas Hesiod made Prometheus' trickery the reason for Zeus' hiding fire and the other goods and so rendering labour necessary, Virgil omits mention of Prometheus or of any element of guilt.

Open peer commentary on the article “Religion: A Radical-Constructivist Perspective” by Andreas Quale. Upshot: The first of my three main goals in this commentary is to demonstrate that Quale’s radical separation between cognitive and non-cognitive knowledge is not viable. The second is to establish Quale’s assertion that a radical constructivist cannot be genuinely religious is a result of taking radical constructivism and religion as abstracted first-order models and is a result of comparing and contrasting elements of these models. The third (...) goal is to establish how religious radical constructivists establish relations between their religious beliefs and their radical constructivist beliefs. To accomplish the first goal, I appeal to the work of Damasio to establish that what Quale refers to as “non-cognitive knowledge” is inextricably cognitive. I also appeal to the work of both Damasio and von Glasersfeld to demonstrate that the Cartesian duality between mind and body is not viable. To accomplish the last two goals, I make a distinction between first- and second-order knowledge and contrast Quale’s argument that a radical constructivist cannot be religious with the relation between religious beliefs and radical constructivism from the perspective of actual religious radical constructivists. The development of neuropsychological assessment methods using virtual reality (VR) is a valid and promising option for the detection of cognitive impairment in the older people, focusing on activities composed of tasks of multiple demands. This study verified the association of age, schooling, and general cognitive status on the performance of neurologically healthy older adults in ECO-VR, a virtual reality task of multiple demands for neuropsychological assessment. A total of 111 older adults answered a sociodemographic questionnaire, the Mini Mental State (...) Examination, the Vocabulary subtest of the Wechsler Intelligence Scale for Adults (third edition), and the ECO-VR. Correlation analyses, multiple linear regression, and comparisons between groups (effects by age and schooling groups) were used to evaluate the results. The ECO-VR total score was significantly associated with age, years of education, MMSE, and Vocabulary subtest. The linear regression models identified that age was the main predictor for total score and rule breaking of ECO-VR. According to the univariate analysis, it was identified the main effect of age group and schooling group in the total ECO-VR score, but there was no interaction effect. The results are discussed in order to understand the role of sociodemographic characteristics in the performance of older adults in a virtual reality task of multiple demands. It was also verified the possibility use of virtual reality for neuropsychological assessment of older adults. (shrink)

Open peer commentary on the article “Ethics: A Radical-constructivist Approach” by Andreas Quale. Upshot: The first of my two main goals in this commentary is to establish thinking of ethics as concepts rather than as non-cognitive knowledge. The second is to argue that establishing models of individuals’ ethical concepts is a scientific enterprise that is quite similar to establishing models of individuals’ mathematical concepts. To accomplish these two primary goals, I draw from my experience of working scientifically with von Glasersfeld (...) for 25 years while he was developing radical constructivism as a coherent model of knowing, and appeal to several of his basic insights to establish constructing models of ethical concepts as a scientific enterprise. A purported axiomatization, by P. Gärdenfors, of intuitionistic propositional logic is shown to be incomplete, and that the mistaken claim to completeness is seen to result from carelessness in the choice of primitive logical vocabulary. This leads to a consideration of various ways of conceiving the distinction between primitive and defined vocabularies, along with the bearing of these differences on such matters as are discussed in connection with Gärdenfors.

Recently, in the framework of a relativistic quantum theory with invariant evolution parameter, solutions have been found for the two-body bound state, whose mass spectrum agrees with the nonrelativistic Schrödinger energy spectrum. In this paper, we study the radiative transitions of these states in the dipole approximation and find that the selection rules are identical with those of the usual nonrelativistic theory, expressed in a manifestly covariant form. In addition to the transverse and longitudinal polarizations of the nonrelativistic theory, we (...) find a “scalar” transition, induced by the relative time coordinate, which is of the same type as the longitudinal transition, expressing the Lorentz covariance of the theory. Patient safety has only recently been subjected to wide-spread systematic study. Healthcare differs from other high risk industries in being more diverse and multi-contextual, and less certain and regulated. Also many patient safety problems are low-frequency events associated with many, varied contributing factors. The subject of this paper is the epistemology of patient safety (the science of the method of finding out about patient safety). Patient safety research is considered here on the background of a risk management framework which requires (...) researchers to: • Understand the context - as a subset of healthcare quality, services and systems research, with technical and human behavioural (cultural) components and a range of external and internal organisational influences, a wide range of research disciplines is necessary • Identify the risks - identify the things that go wrong and the frequency and nature of different types of incidents from sources such as medical record review, observational studies, audit, incident and medico-legal reports • Analyse the risks - deconstruct the things that go wrong, identifying contributing factors and trying to detect trends and patterns in contributing factors, detection, mitigation factors, ameliorating factors and actions taken to reduce risk • Evaluate the risks - decide on priorities, identifying preventive and corrective strategies and judging the risk- and cost-benefit of potential corrective strategies such as standardisation or simplification of a process or device • Manage the risk - evaluate and scope preventive and/or corrective strategies and then implement these, or place the problem on a risk register pending solution, or accept that what is needed is unaffordable • Communicate and consult - use interactive sessions, audit, on-going feedback, reminders and patient mediated prompts • Monitor and review the state of the problem - get baseline trends and patterns so that changes can be tracked and properly attributed to an intervention A hierarchy of levels of evidence has been proposed for clinical research and we argue that insufficient weighting has been given to lower ranked levels of research and to qualitative research, although critical interpretive synthesis is now gaining acceptance in mainstream thinking (e.g. by the Cochrane Collaboration). Fundamental challenges remain including how to grasp the elusive concept of patient safety, how to quantify, characterise and cost the problems, how to judge the extent to which harm can be attributed to errors, violations or system failures, how to identify contributing factors and the extent to which they can be implicated, how to judge whether incidents or their precursors are preventable, how to generate strong evidence to make healthcare safer and how to translate research into practice. Future directions include addressing the mundane as well as rare, dramatic events, and developing further research in non-hospital settings and in developing countries. In summary, a mixture of qualitative and quantitative methods, using information from all available data sources and combining retrospective, real time and prospective study designs, is necessary to address some of the more difficult patient safety problems.
